Q: How do I set an appointment with Professional Family Geriatric Care Management to evaluate my family member’s needs?
A: Just give Jan Sonnenberg a call at 573-893-2878 or send an email to jan@professionalfamily.com. I will set an appointment that is convenient for you and your family. Evening and weekend hours are available. Usually the 1st appointment is used to get acquainted, to gather basic information about the services needed, and to sign the contract and health information privacy documents. Additional appointments will be needed to assess the situation and develop a care plan.
Q: How much does it cost to have Professional Family Geriatric Care Management assist us with our aging family member?
A: Professional services are billed at an hourly rate of $75.00. Jan’s creative use of community and client resources makes her services affordable and can even save clients money in the long term.
Q: Are you like a home health agency that employs caregivers?
A: No, I do not employ caregivers. I find services and coordinate care utilizing the most appropriate providers in your area. I do not contract with any home health providers or long term care facilities, but I do try to develop a good relationship with all providers. I provide constructive criticism or consultation in order to obtain the best service possible for my clients. If needed, I would interview staff to insure compatibility with the client. I will also monitor the situation to be sure that care is given properly.
Q: Why are you called Professional Family?
A: My goal is to provide the professional expertise that comes from years of experience in managing long term care with the family-like relationship that comes from personal attention and really getting to know the client. I can never replace family members who live far away or have other responsibilities, but I can do the things family members would like to have done to make a better life for their loved one. I can also keep family members informed so they feel more connected and involved with the life of their loved one.
Q: Why did you start Professional Family?
A: Even though I had been the administrator of an assisted living facility (Heisinger Lutheran Home) for many years, I found out how hard it was to be a long distance care giver when my own parents and my mother-in-law started needing help. I learned a lot by helping them stay in their own apartments as long as possible. I also learned how much I appreciated those caring people who went out of their way to make life better for my loved ones.
I have always enjoyed working with older adults and I have always enjoyed making a difference in the quality of their lives. By starting Professional Family, I am able to do the things I enjoy and still have time to spend with my own parents on a daily basis.
